For your first visit at our office, please bring the following;
- Current Active Insurance Card
- Policy holder Name, DOB and phone number
- Valid ID
- Current immunization record In state or Out of state.
- Newborn discharge summary and the folder or any testing done at the hospital (for newborn visits only)
- Any kind of School physical forms to be filled out.
- Any medical records about your child past pertinent medical information.
- Copayment or coinsurance amount
- If you have more than one insurance active we will need information for all the active insurances at the time of visit along with secondary insurance cards.
New patient forms are available on link below. You may download and print these forms and fill them out prior to your visit. This will help save your wait time and also assist us in the process of registration.
First Visit: It is very important the patient be accompanied by parent(s) or the legal guardian(s). We obtain important health history information from parents that other adults or the patient may not be able to provide efficiently.
For subsequent visits, if the parent or legal guardian is unable to accompany the child, we require written authorization from parent allowing another adult to bring your child in for medical care.
Please be aware that if vaccines are necessary, this designated adult will be allowed to sign unless otherwise specified.
We collect co-pays at the time of registration. For your convenience we accept cash, check, Master Card, Visa, American Express and Debit cards.
