To provide you with the best health care possible, our office must receive a full copy of your child’s health records including, Immunization including records and a completed copy of our new patient packet before your first visit.
Advance Pediatrics highly encourages parents to immunize their children. Childhood vaccines are safe and save lives. Advance Pediatrics follows the Centers for Disease Control (CDC) and American Academy of Pediatrics(AAP) immunization and catch-up schedules.
We are flexible with parents choosing their vaccine schedule for their child. We carry combo as well as individual vaccines.

Medical Records Transfer
If you are transferring care from another practice, please request that a copy of your child’s medical records be sent to us. You may download the below Authorization to Release Records form for this
Please give this form to your child’s previous healthcare practice and have them forward a copy of your child’s records to our central medical records department at the address below. There should be no charge to you for this service if the records are sent directly to our office at:
